F: Physical Examination Record for Foreigner

(You could download the form from this webiste: http://stuex.nju.edu.cn/en/a/Download_Center/20150805/64.html

(for students who apply for one academic year’s exchange ONLY)

It is highly recommended that students do the physical examination after they come to China. If students do the physical examination in their country, they still need to get the report verified and it seems many reports are deemed unqualified by the Health Center. Therefore, students are strongly advised to do the physical examination in China.

The examination of “Physical Examination Record for Foreigner” could be done in your home country in advance. Exchange students must take the original copy of “Physical Examination Record for Foreigner” by themselves to Nanjing University. Please DO NOT send it to our office.

Application Step – by coordinator

If the coordinator would like to nominate a student to study at Nanjing University, please complete and submit the Online Nomination Form in our Nomination System:

http://stuexdata.nju.edu.cn/index.php?m=Home&c=public&a=login

(We would offer “User ID” and “Password” to coordinator ONLY)

After nominating students online, please submit scanned copies of all above materials of your applicants to stuex@nju.edu.cn. (All documents pertaining to one student must be submitted in one file, with the student’s full name clearly indicated.)

Please note that applications should be sent officially by coordinators of home universities. Personal application is not acceptable.

Please complete all application materials with clear information, including students’ full name, contact details, mailing address, etc. Poor handwriting may delay the admission process.

Insurance

Exchange students must have comprehensive health insurance valid in China for enrollment as a full-time student at NJU. Since many insurance policies purchased outside China require substantial co-payments, have caps or exclude many illnesses. It is therefore urged that all incoming students should purchase a student health insurance when they do registration at NJU.
